How extras accumulate without anybody deciding
It happens through reasonable requests. Somebody asks the cleaner to deal with a spill, clear a store room, or attend an extra day before a visit from head office. Each is small and each is invoiced. Individually none of them is worth querying, and collectively they can add a meaningful amount to a year's spending that nobody planned for and which nobody has any record of having authorised.
The problem is not the charges but the absence of a process. Where requests are made informally by whoever is standing there, nobody has a complete picture, and the first time the total becomes visible is when somebody reviews the annual spend. By then the pattern is established and unpicking it requires a conversation with several people and by whoever happens to be reviewing the account at the time.
